INFORMATION ON THE PROCESSING OF PERSONAL DATA

Based on Article. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as "GDPR" ) I inform that:

You have the right to request from the administrator access to your personal data, rectification or limitation of processing.

You have the right to lodge a complaint with the President of the Personal Data Protection Office.

The recipients of your personal data will only be entities authorized to obtain personal data under the law and other entities with which the Administrator has concluded contracts entrusting data processing for the purpose of providing medical services, providing administrative, IT, billing, and social security services.

Your personal data will be processed for the purpose of providing medical services - pursuant to Art. 6 para. 1 paragraph c and art. 9 section 2 paragraph h GDPR and in order to protect the facility and property, increase security on the premises of the Center and enable the detection of behavior harmful to the Center or exposing it to losses, pursuant to Art. 6 section 1 paragraph f. GDPR via a video monitoring system (image).

I consent to the processing of my personal data contained in the application form by SANTA Sp. z o. o. based in 艁贸d藕 at ul. Wigury 19 for the needs necessary to complete the patient registration process. Providing personal information is voluntary. I have been informed that I have the right to access my personal data, the possibility of correcting it, as well as deleting it and requesting the cessation of its processing.
